Upgraded Puppy/Kitten Pack Conditions
1. Black Hawk Upgraded Puppy/Kitten Pack may change from time to time and may be discontinued at any time.
2. Once you are approved to receive upgraded packs you do not need to complete this form again.
3. Your eligibility may be checked at any time, including when an order is received, and if we feel that you don't meet the criteria then we reserve the right to decline or remove your access to these packs and provide our standard Puppy/Kitten Packs instead.
