I confirm that I have been a Black Hawk MasterBreeder for at least 6 months and that a minimum of 80% of my dogs are fed Black Hawk with it comprising at least 80% of their diet.

I confirm that all my Black Hawk purchases are placed through my MasterBreeders account and understand that they must be purchased through this account to qualify.

I confirm that my litters are ALL raised on Black Hawk.

I agree to order the NEW Upgraded puppy packs for my NEW puppy owners ONLY and understand that this is a trial offering.

I agree to encourage my puppy owners to sign up for the discount voucher off their first purchase of Black Hawk.

I understand that Masterpet/Black Hawk will agree to my upgrade Puppy Pack status if the information I have agreed to above is deemed correct based on my purchases to date.

Conditions: Black Hawk Upgraded Puppy Packs contents and offer may change from time to time. The Upgraded Puppy Packs are a trial only and can be discontinued or changed at any time. Once a MasterBreeder is approved to receive upgraded packs – they do not have to complete this form again. However, their eligibility will continually be checked at any time. If, based on the information provided, Masterpet feel that the MasterBreeder does not meet the criteria, they reserve the right to decline any application without explanation. All MasterBreeders have access to the standard Puppy Packs if they prefer these or if they do not meet the criteria for the Upgraded Puppy Packs. The MasterBreeders eligibility will be checked upon an order being placed for Upgraded Puppy Packs and Black Hawk reserve the right to send the standard packs instead if it is deemed that the MasterBreeder no longer qualifies based on the agreement above.
